When Medicine is Socialized

In 1918, the Soviet Union became the first country to promise universal "cradle-to-grave" healthcare coverage, to be accomplished through the complete socialization of medicine. The "right to health" became a "constitutional right" of Soviet citizens.

The proclaimed advantages of this system were that it would "reduce costs" and eliminate the "waste" that stemmed from "unnecessary duplication and parallelism" — i.e., competition.

These goals were similar to the ones declared by Mr. Obama and Ms. Pelosi — attractive and humane goals of universal coverage and low costs. What's not to like?

The system had many decades to work, but widespread apathy and low quality of work paralyzed the healthcare system. In the depths of the socialist experiment, healthcare institutions in Russia were at least a hundred years behind the average US level. Moreover, the filth, odors, cats roaming the halls, drunken medical personnel, and absence of soap and cleaning supplies added to an overall impression of hopelessness and frustration that paralyzed the system. According to official Russian estimates, 78 percent of all AIDS victims in Russia contracted the virus through dirty needles or HIV-tainted blood in the state-run hospitals.

Socialized medical systems have not served to raise general health or living standards anywhere. In fact, both analytical reasoning and empirical evidence point to the opposite conclusion. But the dismal failure of socialized medicine to raise people's health and longevity has not affected its appeal for politicians, administrators, and their intellectual servants in search of absolute power and total control.

Most countries enslaved by the Soviet empire moved out of a fully socialized system through privatization and insuring competition in the healthcare system. Others, including many European social democracies, intend to privatize the healthcare system in the long run and decentralize medical control. The private ownership of hospitals and other units is seen as a critical determining factor of the new, more efficient, and humane system

SIV mutates and infects human

A woman who tested positive for HIV has been found to be carrying a new strain of the virus which is thought to have originally been passed to humans by gorillas.

The new strain was identified in a 62-year-old woman who moved to Paris after living in Yaounde, the capital of Cameroon. Three other strains of HIV-1 have previously been identified and are all thought to have derived from a similar virus carried by chimpanzees.

Lesson:  Evolution happens.  Strains of SIV related to ones that have crossed over to humans will continue to mutate, and can cross over to humans if given half a chance.  Mutations happen, and will continue to happen.  Some of the changes are bound to be more of a nuisance than others.

Applications of Darwin's Theory

...as scientists mark Darwin's 200th birthday this month, evolution is no longer simply a narrative of life. Scientists have begun using it as a tool to develop new technologies.

By doing so, they have improved law enforcement, created smarter computer programs and are remaking the field of medicine. There have been quirkier applications, such as cleaner clothes, too.

Only recently, though, have scientists begun calling attention to these breakthroughs, as the ideas of the iconic English naturalist have faced renewed attacks.

One of the first and most well-known uses of evolutionary theory has come in law enforcement.

Among the 3 billion or so chemical bits, or letters, in human DNA are those that change slowly and those that mutate rapidly. Scientists realized that short segments of DNA vary greatly from person to person as a result. The finding gave rise to the concept of DNA fingerprints to identify individuals.

Evolution also has helped scientists become smarter about the development and use of medicines such as antibiotics and vaccines.

Physicians now understand that bacteria mutate over time. When attacked by antibiotics, some bacteria become resistant to certain antibiotics and thrive.

"Evolutionary theory has definitely guided us, and now we as a medical community know to be much more careful about the use of antibiotics," said Dr. James Versalovic, a Baylor College of Medicine professor and the director of microbiology at Texas Children's Hospital.

The evolution of viruses also is critical to the development of vaccines. Some viruses, such as polio, have genetic material that changes slowly over time, allowing physicians to create a vaccine that will last for life.

Other viruses, including influenza and HIV, are constantly changing. This requires annual changes to the flu vaccine and has prevented development of a successful vaccine against HIV.

"We know we are in a world where we are in a constant competition with bacteria and viruses," said Dan Graur, a University of Houston biologist. "We need to use evolutionary principles just to keep them under control."

Some scientists also use evolution to develop drugs to combat disease, an approach called "directed evolution."

Biologists employing this method generate a large batch of random segments of DNA- short strips of biological material - which are then tested to see whether any will perform a useful function inside cells, such as binding to a protein. The DNA segments that fail at the task are eliminated; those that succeed are copied in such a way that new mutations are introduced. These new variants then are tested. The process is replicated many times until an optimal sequence is found.

Duseberg and Aids

On 28 October, Robert Willner held a press conference at a North Carolina hotel, during which he jabbed his finger with a bloody needle he had just stuck into a man who said he was infected with HIV. Willner is a physician who recently had his medical license revoked in Florida for, among other infractions, claiming to have cured an AIDS patient with ozone infusions. He is also the author of a new book, Deadly Deception: The Proof that SEX and HIV Absolutely DO NOT CAUSE AIDS. He insists that jabbing himself with the bloody needle, which he describes as “an act of intelligence,” was not meant to sell books. “I’m interested in proving to people that there isn’t one shred of scientific evidence that HIV causes any disease,” Willner says.

Relying largely on the work of Peter Duesberg who claims that HIV is harmless and that AIDS is the result of the use of illegal drugs and drugs like AZT, a number of "HIV dissenters" have been clamoring for attention and trying to effect policy changes.

If they're right, people are getting sick and dying while medical science is offering treatments that either fail to address the disease, or actually make it worse. If they're wrong, people who take their advice are putting their lives, and the lives of their loved ones, at risk.

Tons of research have been done on HIV, including studies that seem to show it entering, hijacking, and killing critical immune cells. We should be able to reach a pretty definitive answer as to who's right. Both sides can't be right here, and whichever side is wrong is a wonderful example of how bad science kills people.

...although the scientific community seems concerned about the effects of Duesberg’s message, with few exceptions—such as Nature editor John Maddox, who took on the London Sunday Times for its AIDS coverage—the scientific community has largely ignored Peter Duesberg. But because the Duesberg phenomenon has not gone away and may be growing, Science decided this was a good time to examine Duesberg’s main claims. In a 3-month investigation, Science interviewed more than 50 supporters and detractors, examined the AIDS literature, including Duesberg’s publications, and carried out correspondence and discussion with Duesberg. This investigation reveals that although the Berkeley virologist raises provocative questions, few researchers find his basic contention that HIV is not the cause of AIDS persuasive. Mainstream AIDS researchers argue that Duesberg’s arguments are constructed by selective reading of the scientific literature, dismissing evidence that contradicts his theses, requiring impossibly definitive proof, and dismissing outright studies marked by inconsequential weaknesses. The main conclusions of Science’s investigation are that:
  • In hemophiliacs (the group Duesberg acknowledges provides the best test case for the HIV hypothesis) there is abundant evidence that HIV causes disease and death (see p. 1645).
  • According to some AIDS researchers, HIV now fulfills the classic postulates of disease causation established by Robert Koch (see p. 1647).
  • The AIDS epidemic in Thailand, which Duesberg has cited as confirmation of his theories, seems instead to confirm the role of HIV (see p. 1647).
  • AZT and illicit drugs, which Duesberg argues can cause AIDS, don’t cause the immune deficiency characteristic of that disease (see p. 1648).

This raises the question: what motivates someone like Duesberg to pursue this theory in the face of such opposition? It seems obvious that he and many of his supporters are True Believers. He has lots to lose by bucking the consensus, and if he's right, he'll win fame, and maybe a bit of a fortune. And why does this alternate theory have any following at all?

The linked article quotes Steven Epstein, "a sociologist of science" at UCSD:

What seems to gives this controversy a lot of its motive force and its peculiar twists and turns is the way in which it’s enacted in very public arenas.

The press gave enthustiastic support to Duesberg's "attack on the AIDS establishment", and people who are at a high risk for AIDS have an emotional investment in any message that tells them they're really safe.

Part of the reason why Duesberg's views are hanging on is that AIDS hasn't been cured yet. In a culture where world-shaking problems are wrapped up in a maximum of eighty minutes (allowing time for commercial breaks), the notion that it might take decades to solve a hard problem doesn't compute. If we haven't come up with answers after a whole decade of work, it must mean the theory we're using is wrong.

Another [reason Duesberg’s message found receptive audiences outside the scientific community] is that his attacks on AIDS researchers as greedy self-interested mythmakers clicked into a growing disenchantment with the medical establishment.

In fact, you see this belief popping up throughout the "alternative medicine" community. One ad I heard recently essentially accused the pharmaceutical companies of making drugs that don't cure diseases. If they actually cured anything, people wouldn't need to buy drugs, and the companies wouldn't make any money. Similarly, one can argue that AIDS researchers would lose all that grant money if they actually cured the disease.

Harold Jaffe, head of the CDC’s Division of HIV/ AIDS, also senses disenchantment with the established order. "In the beginning, it may have represented honest scientific argument," says Jaffe. "Now it has assumed some kind of cult status. It’s attractive to people who believe the establishment is always wrong. This would be the biggest example of all."

It would be interesting to draw the parallels between this "controversy" and the "controversy" over evolution and ID/IOT. For the time being, I'm going to leave that as an exercise for the reader with no life.

Bad theories still kill people

Tech Central Station's James Pinkerton looks at what happens when we deny the fact of evolution. It's an example of when political correctness runs up against the hard rocks of reality:

The first kind of political correctness to take a hit is, interestingly, a p.c. of the religious right: creationism. If some Christians wish to believe that dinosaurs and cavemen co-existed -- because after all, the Bible says that the world is just a few thousand years old -- that's not necessarily devastating. If Christian literalism chooses to deny evolution, that's arguably harmless. But the AIDS superbug, nevertheless, is evolving, and it's evolving according to the immutable principles of natural selection. That is, the killer virus is playing by the exact same rules that have refereed life over the last three-and-a-half billion years.

Creationists have finally given in to the fact that evolution happens. They call evolutionary change which absolutely can't be denied, "micro-evolution". Anything else, they dub "macro-evolution" and declare that level of change impossible. Dogs were always dogs, cats were always cats, and most of all, humans were always humans. Yet there is no "micro- and macro-" evolution. It's all evolution. If you believe there are barriers beyond which change won't happen, you're going to get a very nasty shock one of these days.

Scientists have now found two new kinds of retrovirus, and we've seen a couple of cases where patients have progressed from HIV-positive to full-blown AIDS in record time. These new strains exist, and they've evolved from existing viruses. And they're going to keep doing so, if we give them half a chance. Just as abuse of antibiotics encourages the spread of antibiotic resistant pathogens, the behavior that spreads AIDS encourages the spread of drug-resistant strains of HIV.

Australian scientists have accidentally stumbled upon a simple way to dramatically stimulate immunity to deadly viruses like HIV in what's considered a major discovery in the fight against Aids. <snip> Associate Professor Kent said the researchers had initially set out to devise a test to judge how well an animal's immune system could fight HIV.

They extracted blood from vaccinated laboratory animals and then coated the cells with HIV peptide markers - which tell the immune system a cell is infected by the virus. When they injected the peptide-coated blood back into the animals - to create the illusion the cells were infected by HIV even though they weren't - they found it triggered a huge immune response.

So far, it works in rats and monkeys. Human trials are a couple of years off. The same technique shows promise against other chronic infections. I suspect it might also be effective against some cancers.

I predicted, when AIDS first started appearing, that curing it would wind up teaching us lots of stuff about the immune system – stuff that would lead to cures for all kinds of other ailments. It looks like this is going to be one of those spin-offs.

One thing that's very common in science is that researchers looking for one thing will trip over another, even more important, discovery. History is filled with accounts of this sort of serendipity, which ought to warn people that pouring money into directed research programs may not be the best way to solve particular problems.

Bad theories kill people

I was listening to Dennis Prager's first hour yesterday, and he spent some time discussing the various false beliefs certain groups hold. Among things he cited, a speaker who called the LAPD "the biggest gang in LA City", others who say blacks "fear the police as much as they fear gangs", and an article in The Guardian on Feb. 5 with some disturbing statistics regarding beliefs about AIDS.

According to a study carried out in the United States, 15% of black Americans believe that Aids is being used by the government as a form of genocide against black people. Almost half of them are convinced that the HIV virus is man-made, more than one quarter that it has been produced in a government laboratory, and 12% that it has been created and spread by the CIA.

Other theories about AIDS include the belief that the HIV virus is not the cause (Robert Mugabe famously believes this one); that the government is withholding a cure from the poor, and that the drugs used to treat HIV are actually part of a plot to use Blacks as guinea pigs.

The African-Americans who like to think that Aids is a government conspiracy against them have reason to hope this is the case. They must long for a comforting explanation, other than poverty, as to why it is that the disease affects them so disproportionately. They represent 13% of the American population, yet account for 50% of new HIV infections.

That's all well and good, but what is the result of believing a bad theory?

Remember, a theory is not a guess, or an unproven fact. A theory is a structure which purports to organize and account for observed facts. A good theory will account for lots of facts in one neat structure, and will accurately predict other facts. A bad theory will predict things that are not factual. The alternative to a good theory which predicts true things is not a blank slate, it's a bad theory, which predicts falsehoods.

What's more, what theories you believe affect how you will behave and what choices you will make.

If you believe that the police are as dangerous to you as the local gangs are, you will not call the police in to deal with gangs or other criminals. You will either sit there and let yourself be victimized by gangs, in the belief that at least that way, you're only being set upon by one group of thugs, or you will take matters into your own hands and try to deal with the situation yourself. This puts your own safety at risk, and you may endanger some innocent third party.

If you believe that AIDS is spread by the CIA, or that the government has a secret cure, or that it's not caused by HIV at all, you will not change those behaviors that put you at risk for contracting the virus.

If you believe poverty is due to a racist society and not due to how much you work, you will be poor all your life.

Lots of people believe, or disbelieve, particular theories because it makes them feel better. This leads them to act as if certain things are true, which are, in fact, false. At best, this will do them no good; at worst, it can kill them.
